The pun is lost in translation, but I'll try to explain. Japanese has two alphabets, one for Japanese and the other for foreign languages. The title is "nicochin", which is the adaptation of "nicotine" into Japanese, but is spelled using both alphabets. The part in Japanese is "chin", which is short for "chinchin"(penis).
