I don't get how this is NTR, I mean sure protagonist likes her but it's just a one-sided love. She doesn't even acknowledge the protagonist as a love interest.

Its true this is not NTR but the way the story is set up it makes you root for the MC even though he is getting the short end of the stick

NTR is exactly this. The feeling of losing something that you love. Yes, the love was technically unrealized. However, it doesn't change the feelings that were invoked. Definitely NTR-esque.

I agree. What matters is the point of view of the reader, not the actual act of cheating. That's why many works that involve rape go under the NTR tag. For this it's quite amusing seeing how many people get furious and almost desperate when they read it, invoking a ton of supposedly moral protests to console themself. It's a proof the genre is wonderfully effective and, for this very reason, misunderstood. Fascinating, isn't it?
